Direct fiberoptic vocal cord mapping
The anterior commissure of the vocal cords (Broyles' ligament) attaches directly to the inner surface of the thyroid cartilage just below the prominent superior notch. Blindly shaving too far downward can destabilize the vocal cords and permanently lower or hoarsen the voice. We utilize real-time transnasal/laryngeal fiberoptic endoscopy and needle transillumination to mark the exact millimeter boundary of the vocal cord attachment before removing a single millimeter of cartilage.
- Real-time endoscopic visualization of the anterior vocal commissure
- Strict 2 to 3 mm structural safety margin preserved above Broyles' ligament
- Zero risk of vocal fold detachment, pitch drop, or chronic hoarseness
- Full protection of the superior and recurrent laryngeal nerves
